A special surgical case was recently successfully performed by the Obstetrics and Gynecology Department of Hoan My Da Nang Hospital for a female patient of South African nationality suffering from large endometriosis accompanied by large uterine fibroids. The condition had caused prolonged pain and significantly affected her quality of life.

A Complex Case with Multiple Challenges

The patient was admitted with persistent dull lower abdominal pain, accompanied by menstrual irregularities and pelvic pressure. Clinical examination and diagnostic tests revealed a large uterine fibroid measuring 7 cm and an additional 2 cm fibroid classified as FIGO types 4 and 5, along with a suspected endometriotic mass measuring over 10 cm, occupying nearly the entire pelvic cavity.

This was a rare and complex case, as large endometriotic lesions often cause extensive adhesions involving adjacent organs such as the intestines, bladder, and pelvic wall. In addition, as the patient was an international patient, comprehensive consultation, multidisciplinary coordination, and thorough surgical preparation were required.

Laparoscopic Surgery: A Safe and Effective Solution

With the aim of maximizing fertility preservation and shortening recovery time, the Obstetrics and Gynecology team, in collaboration with the Anesthesiology and Intensive Care team, developed a detailed treatment plan, including preoperative medical management with Visanne for six months, prior to performing laparoscopic surgery to completely remove the endometriotic lesions and uterine fibroids.

The two-hour laparoscopic surgery was performed under the leadership of MSc., Resident Physician Ho Thuyen, with the support of anesthesiologists, nurses, and medical technicians. Thanks to extensive experience in managing complex gynecological cases, the surgical team successfully removed all lesions while preserving the anatomical structure and function of the reproductive organs and surrounding tissues.

Postoperatively, the patient recovered well, was able to walk gently after one day, and was discharged after several days of treatment.

Preservation of Reproductive Function After Surgery

With careful preoperative assessment and thorough preparation, the medical team was able to completely remove the endometriotic lesions and uterine fibroids while preserving the right ovary. The left ovary remained in good condition, allowing the patient to maintain normal reproductive potential should she wish to conceive in the future.
